Out-of-school girls are among the most disadvantaged adolescents in rural Upper Egypt. Compared with girls attending school, they are more likely to be engaged in poorly paid farm work, more likely to be married early, and at greater risk for early childbearing and poor pregnancy outcomes. To respond to their situation, the Ishraq program was designed: a holistic intervention to address the unmet needs of out-of-school adolescent girls. The pilot phase of Ishraq was launched in four rural villages of one of the country's poorest regions through the partnership of Caritas, the Center for Development and Population Activities, the Population Council, and Save the Children. This manual was developed from the experiences of a number of communities and organisations in southern and eastern Africa in creating Junior Farmer Field and Life Schools as a response to the growing number of orphans and vulnerable children. For many reasons these children more likely than other orphans to be at risk from malnutrition, disease, abuse, stigmatisation and sexual exploitation. The risk of sexual exploitation is particularly significant for those left alone to cope with poverty and who are forced to adopt adult roles and ensure food for the rest of the family. As parents and family members become ill, children take on greater domestic, agricultural and income generating responsibilities. HIV and AIDS has a particular impact on girls who are left to care for ailing parents, or who have to become the heads of households upon the death of caregivers. Also, as many parents are dying at a young age orphaned children are growing up without the necessary knowledge and skills for their future livelihood

Expanding pediatric access to antiretroviral therapy in South Africa

This paper presents the results of a rapid situational analysis of the antiretroviral (ARV) rollout in South Africa which found that several ARV programmes are treating children successfully. However, all the institutions surveyed identified a large number of concerns and challenges that need to be overcome in order to improve care for children living with HIV. Key actions required include early identification of HIV-infected children, effective referral, standardised training in pediatric HIV management for health professionals, and increased community awareness and support

The changing context of sexual initiation in sub-Saharan Africa

During the past 20 years, substantial reductions have occurred in the proportion of young women who report marrying as teenagers in sub-Saharan Africa. An oft-stated consequence of a delay in age at marriage is a rise in the proportion of young women who engage in premarital sex. This paper investigates the links between changing age at marriage and premarital sexual behavior in 27 sub-Saharan African countries in which Demographic and Health Surveys were conducted between 1994 and 2003. Using multiple-decrement life tables to examine the competing risks of premarital sex and marriage without prior sexual experience, we answer the largely unaddressed question of how reductions in the prevalence of early marriage have affected the likelihood of initiating premarital sex. Our analysis reveals that although the age of first sexual activity has either remained the same or increased, a shift in the context of sexual debut from marriage to before marriage has taken place in many countries. We assess whether the increase in the proportion of young women who report premarital sex is influenced by an increase in exposure resulting from delayed marriage or by an increase in the rate of premarital sex. The evidence on this point is mixed; in some settings greater exposure explains more of the increase, whereas in others an increased rate of premarital sex dominates

Peer education (PE) aims to improve pupils' knowledge, attitudes and behaviour about HIV and AIDS and reproductive health. GTZ supports PE in over 200 schools in the Tanga region of Tanzania. A study was done in schools with and without PE to determine if PE has an effect on pupils' knowledge and attitudes. PE made a difference - pupils have better overall knowledge and are better informed on key facts about HIV and AIDS, and many more pupils with PE considered abstinence a viable option. But even in schools with PE there are some gaps, e.g. many pupils believe that one can see if someone is HIV positive and most pupils have negative attitudes about condoms. Contact teachers were found to share a few misconceptions about HIV and AIDS

Disability, poverty, and schooling in developing countries : results from 14 household surveys

This paper analyses the correlations between a young person’s disability, the economic status of their household, and their school participation. The survey was conducted using 11 household surveys in nine developing countries. The results show that some youth with disabilities live in poorer households, but the extent is not statistically significant. However, young people with disabilities are often less likely to start school and show lower transition rates. This finding suggests that, in developing countries, disability may lead to long-run poverty since youth with disabilities are less likely to achieve qualifications which would allow them to earn higher incomes in their later life
